TMJ jaw pain and night-time teeth grinding, eased

R.B. · A common presentation across many patients

This case was written by Dr. Laura Cowburn, Doctor of TCM and Registered Acupuncturist in Alberta.

At a glance

Aching, tight jaw and night-time grinding, released by clearing the Qi blockage around the joint — settled within a single treatment course.

Presentation

This is one of the most common complaints we see at the clinic. Patients arrive with aching, tightness, and tenderness around the jaw joint (the temporomandibular joint, or TMJ) — the hinge just in front of each ear. Many also grind or clench their teeth through the night without realising it, a habit known as bruxism, and wake with a sore, fatigued jaw.

Left unaddressed, the tension tends to compound: the jaw feels locked, the range of movement narrows, and the grinding wears on both the teeth and a good night's sleep.

Diagnosis

From a TCM perspective, the pattern is one of Qi blockage and muscle tension gathering around the TMJ. When Qi — the body's circulating energy — cannot move freely through the muscles and channels that cross the jaw, the area becomes tight, sore, and prone to involuntary clenching. Stress is a frequent driver, since it constricts the smooth flow of Qi and settles into the jaw as one of the body's favourite places to hold tension.

Treatment

Treatment focuses on releasing the blockage around the jaw and calming the system that drives the clenching, using a small, well-chosen set of points:

  • Linggu (Master Tung's Ling Gu) — a powerful point on the hand, in the web between thumb and index finger, used to move Qi and relieve pain at a distance from the jaw itself
  • LI-4 (Hegu) — the classic 'command point' of the face and head — pressing here helps release tension throughout the jaw and facial muscles
  • a modified Linggu approach — adapted to the individual patient where the standard points need fine-tuning

Because the jaw responds quickly to distal points like these, many patients feel the area soften within the session itself.

Result

The jaw joint stops feeling tense and sore, range of movement through the jaw improves, and the night-time tooth grinding settles. With the underlying tension released, patients are able to open, chew, and sleep without the familiar ache returning.

This reflects an individual patient experience, not clinical trial data.
